Before offering any approval option, exiting plan mode now checks the plan text against a 200,000-character cap and an elision-marker test. If the plan fails that check, every "Yes..." option (clear-context, bypass-permissions, auto mode, accept-edits, manually-approve) and the shift+tab hint are dropped, leaving only "No, keep planning," and the dialog explains why approval is withheld. This applies to everyone.
