Permission dialog fails closed when tool parameters cannot be rendered

If rendering a tool-use descriptor throws while building a permission dialog, the dialog is now shown anyway, marked toolUseRenderFailed, with the message replaced by text telling the user the parameters could not be rendered and to deny unless the action was expected. Previously such a throw propagated and presumably broke the dialog.
