What
A permission prompt asks you whether Claude may use a tool. When you accept one, Claude Code's record of the decision now includes a scope. The decision counts as permanent only when you chose "always". Before, it counted as permanent whenever any rules were saved. When you chose "once" while not in the default permission mode, the scope is recorded as session.
Why
Usage records now tell approvals for one session apart from permanent ones.
