What
Plan mode is where Claude works out a plan before changing anything, then uses a tool to leave plan mode and show you the plan. When that tool is available, the reminder Claude gets in plan mode now notes that the plan goes to it. If that changes during a session, Claude gets the full reminder again.
In some cases Claude Code also no longer reads the plan from the plan file, where it used to always do so.
