Placeholder text in the prompt box now lines up with the cursor

The prompt box can show text that is displayed but not part of what you typed, such as a placeholder standing in for pasted content. Claude Code now draws that text together with highlighting and the cursor, and keeps track of how positions in your draft match positions on screen. Before, only highlighting was handled.
