What
Before the Write tool creates or overwrites a file, Claude Code checks whether it is allowed. If that check itself fails to finish, Claude Code now asks you for permission. In plan mode, where Claude only plans and does not change files, it refuses instead. The message explains that the check did not finish, so its safety checks did not run.
Why
A crash in the permission check can no longer let a write through without its safety checks.
