What
A permission mode controls how much Claude Code may do without asking you first. When a change of permission mode is refused, Claude Code now records the refusal in its usage reporting. When the refused mode is auto mode, the message returned is "Cannot set permission mode to auto".
Why
A refused mode switch is now visible rather than passing silently.
