Moved synced plugins are found and loaded from their new location

Plugins are add-on packages that extend Claude Code, and some are synced into place. If a synced plugin's folder no longer exists, Claude Code now checks the sync manifest, a record of where synced plugins were placed, for the folder that replaced it and loads the plugin from there. Before, it skipped the plugin with a "Plugin path does not exist" message.
