Unclear It is not clear whether plan mode newly forces these prompts in this version or was already doing so and is only now named in the message.
What
A hook is a command of your own that Claude Code runs at set moments, and it can approve a tool call. A tool call is Claude asking to run a command or edit a file. Sometimes Claude Code still shows you the full permission prompt even when a hook approved the call. The message that explains this now lists plan mode as a possible cause, alongside a safety check or a host's ask that only a person can answer.
Why
If you are in plan mode and a hook's approval seems to be ignored, the message now says that plan mode can be the reason.
