A tool call is a single action Claude takes, such as running a command. After a restart, Claude Code records how it handled remote tool calls that were in progress. That record already counted calls that were written down, calls that could be picked back up, and calls that were. It now also counts calls that were served but never sent.
Why
This shows that a remote tool call can be recorded but never delivered, and that restart handling now keeps track of those calls.
