What
When Claude works on a request, it often uses several tools in one turn. A tool is an action such as reading a file or running a command. Claude Code can now gather a list of the earlier tool calls in the current turn whose results came back as errors.
If building that list fails, Claude Code records the failure in its internal usage reporting.
Why
This looks like preparation for handling later tool calls differently when an earlier one in the same turn has already failed. Nothing in this release shows it changing what you see yet.
