Unread file references can now say the file could not be examined

A referenced file that was not read now shows "not read: could not be examined" when it could not be examined, and "not read" otherwise

When you reference a file and Claude Code does not read it, the line shown for that file has new wording. It now says "not read: could not be examined" when the file could not be examined, and "not read" in other cases. It used to always say "(not read)".

Why

The line now tells you whether a file was skipped or could not be looked at.
