Teammates get their agent definition from the request that starts them

When Claude Code starts a teammate agent, it now uses the definition sent with the request instead of looking it up by type

Teammates are extra agents that run alongside the main one inside the same Claude Code process. When one is started, Claude Code now uses the agent definition sent with the start request, and works this out earlier in the process. Before, it looked the definition up by agent type in the list of active agents. It also writes a debug line naming the agent type.
