When Claude wants to do something that needs your approval, Claude Code shows a permission prompt. Some prompts offer an "always allow" choice so you are not asked again. If the request is for a domain, meaning a website address, that is already allowed, the prompt no longer offers "always allow".
Why
An "always allow" choice would add nothing for a domain that is already allowed, so you may no longer see it on those prompts.
