What
Claude Code keeps a list of error messages that it treats like a cancellation rather than a real failure. That list now also includes "Tool permission stream closed before response received". This is the error when the connection that carries a permission answer closes before the answer comes back. A permission answer is your yes or no on letting Claude use a tool.
