What
When the classifier, the model that judges whether a tool call is safe, refuses a call, Claude Code gives Claude guidance on what to do next. That guidance changed. Claude is now told to retry once if still needed, and if it is refused again, to stop and tell you that the tool needs your own approval.
Before, Claude was told to send the call again and ask you to approve it from the terminal or desktop prompt.
Why
This can keep Claude from sending the same refused call over and over.
