When the working folder changes, Claude Code now finds the permission prompt tool's MCP server from the listed tools, not only from its parsed name

An MCP server is an add-on program that gives Claude Code extra tools. A permission prompt tool is one that answers permission requests in place of you. When the working folder changes, Claude Code clears out MCP servers that no longer apply. It now works out which server the permission prompt tool belongs to by looking at the tools that server lists, as well as by the server name it parsed. Before, it used only the parsed name.

This fixes a case where the permission prompt tool could be missed after the working folder changed.
