Calls to MCP tools whose server is still starting now wait instead of failing

MCP servers are add-ons that give Claude extra tools. They connect when a session starts, and some take a moment. Before, if Claude called a tool whose server had not finished connecting, the call failed at once with a "no such tool" error.

Now Claude Code checks whether the name belongs to an MCP tool whose server is still connecting for the first time, or which has just appeared in the refreshed list of tools. If so, the call is queued instead of rejected. A server that is reconnecting does not count. If Claude Code cannot tell, it logs a warning.
