What
Claude Code had a fallback that located files by their path name when anchoring certain file operations to a held folder. That fallback has been removed. These operations now either use an open file handle, which points directly at the file, or are reported as unavailable.
Why
On systems that used the path-based fallback, these file operations will no longer be done by path and may now be reported as unavailable instead.
