Clearer startup message when gateway settings cannot be applied

Some organisations deliver Claude Code settings from a Cloud gateway, a central server that hands out managed settings. When those settings are required but none of them can be used, Claude Code stops at startup and prints a message. That message now opens with a plain sentence saying the settings could not be loaded from the gateway. Before, it opened by saying none of the settings the gateway served could be applied as written, and ended by asking you to have your administrator fix them.
