When you resume a session (pick up an earlier conversation), Claude Code rebuilds its record of which files were read from the transcript, the saved conversation history. For a message holding a single result, it now takes the file content straight from the tool result instead of parsing the text that was shown on screen.
Why
The rebuilt record uses the actual file content, not a version that has been formatted for display.
