A recap is a summary of what happened in a session. When Claude Code cannot produce one, it still shows "Couldn't generate a recap. Run with --debug for details." The result behind that message is now also marked with an outcome of failed.
Why
The failure is now recorded as such rather than existing only as a message. Nothing changes in what you see.
