Before Claude Code runs a tool (an action such as editing a file or running a command), a permission check decides whether to allow it, deny it, or ask you. A new pre-check can now change an allow into an ask. The recorded reason then includes reroutedFrom and replaces.

This applies only when the call is not a servedCall and the current permission mode passes a check.

Why

In practice, some actions that would have gone through automatically can now stop and ask for your approval.
