The placeholder tool object built for a tool name that isn't currently loaded now derives isMcp from the name's mcp__ prefix instead of hardcoding it to false. Tools named with that prefix are now classified as MCP tools in permission rendering and rule matching even when the real tool object hasn't been loaded.
