Sibling tool uses from the same turn are now always folded into the messages passed to permission evaluation.

- Previously
sameTurnToolUses was included only when a gate function backed by CLAUDE_CODE_AUTO_MODE_SIBLING_CONTEXT returned true. - That gate and its environment variable are both removed from the build.
