MCP Server Input Request Shows Task ID

When an MCP server requests user input, the dialog title now includes a truncated task ID (e.g. MCP server "foo" requests your input (task abc12345)) to help distinguish between multiple concurrent requests.

Task ID in MCP input dialog (visible in ZP5 structural change appending task ID to title)
