The most common causes are an LLM gateway or proxy that drops the request for review or the results, and a platform, region, or credential that doesn't have server-side checks yet. Claude Code falls back to its own classifier requests. On a direct connection to the Anthropic API, the variable requires Claude Code v2.1.281 or later. Setting it to `1` there turns server review on in a session that doesn't have it yet, such as a `-p` or Agent SDK session, unless you've also set `CLAUDE_CODE_DISABLE_EXPERIMENTAL_BETAS=1`. If you set `CLAUDE_CODE_DISABLE_EXPERIMENTAL_BETAS=1` and leave `CLAUDE_CODE_AUTO_MODE_SERVER` unset, Claude Code also stops asking the server.
