A tool call is one action Claude takes, such as reading a file. Each cleared result is replaced with a short placeholder.\n\nBefore, a result containing images or documents was swapped for the placeholder with only a short marker in front. Now the text at the start of the result is kept and placed before the placeholder, unless the placeholder already begins with that text.
