Before, there was a single slot for one prompt, cleared when it closed. Now several prompts can be open at once:\n\n- Prompts are opened with `openAsk`, shown with `showAsk` and closed with `closeAsk`. Each prompt carries an order and can be marked to go on top.\n\n- The prompt at the top of the stack is the one displayed.\n\n- When a prompt is asked again, a notification is also sent.\n\n- Separately, messages saying a host is unresponsive now include a note about its output, and a standing status line is shown.\n\n**Why**\n\nThis may change how several approvals waiting at the same time are shown.
